**Rates are hourly, please select the number of hours you would like to use for a supervised visitation, for example, select 3 for three hours or 4 for four hours*** Please confirm the date with your supervisor no later than 48 hours prior to the visitation date.***
Key Policies:
Arrive on time for visits.
Notify Bundle Therapy 24 hours in advance if unable to attend.
Visits are conducted at specified locations only.
No discussions of adult matters in the child's presence.
Bring photo identification to each visit.
Only authorized persons may participate in visits.
Visits will be canceled if substance abuse is suspected.
Positive parenting practices are required; no corporal punishment or profanity.
Respect Bundle Therapy staff and follow all rules.
All conversations must be audible to the visit facilitator.
Parents are responsible for their child's behavior during visits.
No weapons, animals, or pets allowed.
No administration of medication to the child during visits.
Compliance with all rules is mandatory to continue visitation services.
